Our story
Well & Care Co. was created to address a common gap in disability support: workers, families and allied health professionals may all care deeply about the same participant, yet not always have a shared understanding of goals, observations or progress.
Our service model brings greater structure and connection to everyday support. Clear notes, thoughtful handovers and open communication help daily routines reinforce participant goals, identify risks earlier and respond to changing needs.
We are building a company where every shift has purpose, every team member understands their role, and participants receive consistent support focused on meaningful outcomes.

We want every shift, note, conversation and care decision to contribute to safer support, stronger independence and a life shaped by the participant's own goals.
Our three commitments
We believe strong documentation is part of good care. Formal intake, service agreements, risk assessments, shift notes and incident processes create clarity and protect participants, workers and families. Well & Care Co. is preparing for formal NDIS registration and intends to develop its governance framework with specialist provider-registration support.
Support workers should understand more than today's task list. With the participant's consent, we work closely with families and allied health professionals so therapy goals, changing needs and practical observations can inform everyday support and be tracked over time.
Consistent care depends on workers who feel capable and valued. We are committed to practical training, clinical mentoring, clear expectations and wellbeing support that reduce burnout and help strong relationships last.
